Admission
to Old Peachtree Montessori School requires the
following:
• An application with application fee
• An evaluation/observation session, conducted by one of our teachers
• Contract
• Student Information Addendum
• Employer Information
• Student Behavior Policy
• Attendance Policy
• Immunization Form 3231
• Parents in Action Volunteer
• Materials Fee of $250 per child
• Email address
We must obtain the following parental authorization when a child is enrolled.
• Authorization for the center to obtain emergency medical care for the child when the parent is not available
• Authorization for the center to dispense medication
• Authorization for the child to participate in field trips and special activities away from the center (Elementary only)
• Authorization for persons other than the parents to pick up or drop off their child, including the names of such persons
